3 AOF/ Smart Health Project Definition Financial Resource: Contract SMART HEALTH CENTERS PROGRAM, Civic Federation funded by several local Foundations. Project Scope: Develop and Implement Version 1.01 of System of Health Information SMART- HEALTH-COMPUTER STATIONSs for Participating Churches--- for Congregants and Community Residents: EXCLUDED from this Project and scheduled for a subsequent effort will be A System for a Virtual User Community with Semi-automated Cyber-Navigation assistance, including 24/7 on-line Help Primary End-Users: Congregants ages 12 and above who are assisted by ‘Cyber-navigators’: Users will be provided with on-screen experience and information that aid in taking specific actions (ex. Make to-do list for making Good Health a personal priority, Following up on specific actions for Healthy life-styles, Making informed access to health care system.) Policy and Environmental Concerns: ALL CONTENT IS TO BE FROM AOF-APPROVED ESTABLISHED RESOURCES such as Government Agencies (Federal, State, County)), Associations (ex. Heart, Diabetes, Stroke, Arthritis, etc.), Medical Societies, Selected professional and Health resources, such as PTSC, Restart4Health, MEND, etc. NO MEDICAL CARE RECOMMNENDATIONS ARE TO BE MADE. No substitutes for Medical Care are to be offered. Motivation approaches will be established and implemented in the Participating Churches to encourage use of the SMART-HEALTH-COMPUTER STATIONSs. SMART-HEALTH- COMPUTER STATIONS use will be without fees. User privacy will be protected totally. Project Timeline: TBD and posted on Project plan and schedule page.: Target Date for Completion of delivery, testing, orientation, installation and implementation in participating Churches by 3/31/ 2014

14 System Design Parameters Central Server will provide information to computer- based SMART-HEALTH-COMPUTER STATIONSs in all participating churches Central Server will receive information from AOF- approved sources, AOF Curriculum designers will have the capacity to provide supplemental definitions and illustrations where needed --- to be incorporated in feed to Church SMART- HEALTH-COMPUTER STATIONS. Church SMART-HEALTH-COMPUTER STATIONS will provide means for use by Users without tracking of individuals. Tallies will be kept on the materials accessed without identification of which individuals accessed the materials.

15 Levels of System Testing Standard Testing Protocols will be used to test each subset of Code, integration of Modules, Human factors testing, and controlled test of predefined queries. Volume tests will be conducted Scripted Simulations testing will be conducting with each church and throughout the network Random user-simulation testing will be conducted with participating churches Attack simulations will be conducted to test Security features.

16 Implementation Pilot Implementations will be conducted with Small, Medium, and Large congregations and/or with mixture of church sites with Wireless viz. Wired Internet Connections. The Second Phase-in of SMART-HEALTH-COMPUTER STATIONS services will be to Congregants in remaining churches ---- with extra support available for handling customer service issues. The Third Phase-in of SMART-HEALTH-COMPUTER STATIONS services will be to the Communities at large for all churches. A final Acceptance review will be conducted by AOF Board and Working Group Committee aided by an outside advisory panel from professional collaborators.
