Methodology What was the process? Discussed with line manager Carer’s and colleagues provided a list of relevant services Contact made with each service to provide details Leaflet designed and re-designed

Methodology Audited via brief questionnaire distributed to carers (Feedback positive !) Distribution approved by line manager and directorate manager Liaison with GP surgeries, psychiatrists, hospitals, voluntary and social care agencies for distribution 100 leaflets printed initially Amendments made and leaflet reprinted
