Approach to Family Outcomes Use NCSEAM survey section, “Impact of Early Intervention Services on Your Family”. Service Coordinators provide surveys to families at each annual IFSP and near exit. Service Coordinators responsible for inputting demographic data. Surveys are available in English & Spanish

The rights for your child and family include the right: To choose whether you want to participate in early intervention. To be an active member of the early intervention team working with your child. To receive a written copy of the AzEIP Procedural Safeguards for Families Booklet and a verbal explanation of these rights in your language or in your way of communication when: AzEIP Family Rights Conundrum Parents report that they know their rights, however they are not availing themselves of the process.
