Standardized heading used on the Decision, Issues, and Action Logs

Decision Log – Is used to track decisions being made on the project. The log starts with giving the decision item an identifier, the date that an issue arise needing a resolution, the date decision has resolved the issue, description of the decision, owner of the decision. If communication to all project parties is needed, if a change is needed and priority of the decision.

Issue Log – Is used to track issues, the dates issue opens, date projected to issue needs to be resolved, the resolve date, what type of issue, description of issue, issue’s impact and alternatives considered or researched. If escalation is needed, identifies (owner to follow-up) if different from project manager, status of action to resolve the issue and if applicable links issue to “Work Breakdown Structure (WBS)” and priority of the issue.

Action Log – Tracks dates actions identified, projected due date for action, date action resolved issue, the type and description of action, impact to project, alternative solution to research. If escalation is needed, if communication to other parties is needed, whose responsible for action item if different from project manager, status of action taking place, if applicable tying back action item to the “Work Breakdown Structure (WBS)”, and action’s priority.

To summarize, these logs are important to your project: It’s a journal of key project activities. It keeps you organized & up-to-date. It provides a “quick status review” of where all the “moving parts,” are headed, and it quickly identifies potential “hot spots,” that need to be monitored and managed.
