09/13/2007 Primary Referral Sources Hospitals, including prenatal and postnatal facilities; Physicians; Parents; Day care programs; Local educational agencies; Public health facilities; Other social service agencies; and Other health care providers.

09/13/2007 Timelines Once the public agency receives a referral, it shall appoint a service coordinator as soon as possible. Within 45 days after it receives a referral, the public agency shall: 1. Complete the evaluation and assessment activities, and 2. Hold an IFSP meeting.
