Developing Our Service Package(s) (Minnesota)

Service packages Major services/service packages that will meet our customers’ needs: Evidence-based programs (CDSMP, MOB, Healthy Moves, Savvy Caregiver, EnhanceFitness, Healthy IDEAS) – Need to determine which interventions are group-based vs. individual – Also determine which network organizations are hosting (i.e. bearing cost) vs. just serving as implementation sites – Also investigate feasibility of offering HomeMeds Home safety assessment – Better define what is meant by this – includes safety as well as bio/psycho/social assessment? Advanced care planning Care transitions (are there various service packages that can be developed to meet specific client needs?) Home care (especially following end of Medicare home care) Major services/service packages that will meet our contracting org’s needs: Same as those that meet customers’ needs? 2

Insights Our biggest insight(s) from this session on Developing Service Packages is/are… Be prepared to offer both short term and long term interventions Re-package services based on need Figure out how to articulate that services offered decrease health care utilization (address “pain points” of various customers) Re-tool existing business in order to better serve/continue to serve current clients as well as engage new client base 8

Action Steps The action step(s) we will take in the next month to develop our service package(s) are: Flesh out service competencies and strengths of each of the 13 network partners Begin to learn more about how our service competencies relate to various health care-related measures (HEDIS, STARS, etc.) Startup cost evaluation re: infrastructure (software, training, IT, consulting, etc.) 9
