Integrated Approaches to Localities

Together Aim is to implement three new locality areas across the city, supporting an alignment with Better Care, Adult Services, NHS Solent, Primary Care and Midwifery Services. Support an improved Early Help Offer that reflects the contribution all services can and do make already. Develop an expanded Headstart offer (phase 3). Where achievable, integrate services to improve our offer.

Early Help Aim is to implement three new locality teams (West, Central and East) Service will reconnect with Families Matter and open pathways for referrals currently not available to partners eg; Police, Schools, Housing. Work more closely with schools to better support the attainment, aspiration and attendance of young people Support families return to work – with strong links into DWP and City Deal

Children’s Centres Aspiration is to integrate with Health Visiting. 3 new localities rather than four (clusters) Support families with children who are entitled to, to access 2 Year Old Funded Early Education and eventually 3 and 4 year olds. Ensure that families with children aged 0-5, who are open to our teams are registered with their local Children’s Centre

School Attendance Implementing a School Attendance Action Plan with schools Ensure that ‘school attendance matters’ and is at the forefront of all of our practice. Work more closely with schools and education colleagues to develop improved packages of support, that can reduce exclusions and deliver better outcomes for children Again in the context of Locality Working
