BACKGROUND We’ve been focused on outreach, enrollment and retention services for over 15 years Formerly the Sacramento Children’s Health Initiative (CHI) for the Healthy Kids Program Launched Healthy Kids Healthy Future, the state’s first regional CHI (Yuba, Colusa, Placer, El Dorado and Sacramento counties) Funded in 2013 by Covered CA & Sierra Health Foundation to conduct region-wide outreach and enrollment services Selected for the first and second Covered CA Navigator Program grant for Northern CA (mostly Sacramento region) Our work includes navigation services to connect individuals to medical homes

PROGRAM OVERVIEW Seven (7) Patient Navigators placed in five (5) local hospital emergency departments (Dignity & UCD) Referral relationship with Kaiser Permanente EDs Three (3) Health Navigators focused on families with children, 0-5 (Telephonic & in-person at UCD) Six (6) Health Navigators focused on health insurance enrollment and access to care One (1) Health Navigator stationed at the Oak Park Community Center to facilitate connections to health and other community resources Multilingual call-center (50+ hrs/week) where information about the new program can be accessed telephonically Hospital ED data sharing relationships - could demonstrate utilization of emergency departments for this patient population

PROGRAM GOALS 1. Ensure patients can access primary care 2. Decrease dependency on EDs for primary care 3. Improve quality of care 4. Reduce uncompensated care costs

NAVIGATION SCOPE OF WORK Connect with patients utilizing the ED for primary care Help reconnect patients to their primary care physician Provide education on benefits for patient’s assigned health plan Encourage patients to use available resources through their health plan Emphasize to patients the importance of preventative care and appropriate use of the ED Provide follow-up for patients with appointments Connect patients with other community resources as needed
