Our goals for our MFP monthly webinars are: To provide training on key topics To better the lines of communication between you and us, the MFP staff To seek feedback on how to better the processes of MFP/waiver services for our participants To provide the opportunity to share successes and challenges as RTC’s and MFP transitions Goals of our monthly webinars 2

MFP Transition Numbers As of February 11, 2016 248 referrals have been received 76 participants have transitioned to date (15 in 2014 and 53 in 2015 and 8 so far in 2016) * The transition benchmark for 2016 is 77 transitions. 3

COVERED SERVICES MFP Modify Existing Vehicle for Accessibility service includes the following: Vehicle Access Modifications – power door operators, wheelchair lifts, raised roof systems, raised door openings, lowered floor (passenger, driver, or both) Steering System Modifications – steering devices, reduced effort systems, emergency backup systems, removed/disconnected air bag Secondary Controls – electronic control touch pads, parking brake extension lever, power parking brake, left hand gear selector lever, power gear selector Brake/Accelerator Systems – mechanical hand control, powered brake/throttle control, left foot accelerator, reduced effort brake, pedal extensions Driver Position – removable driver seat, power transfer seat, transfer assist Miscellaneous – 4-point wheelchair tie-down, battery protector/backup system, wheelchair/scooter hoist, suspension system modifications This list is offered as an example and shall not restrict additional modifications that are requested and approved in a participant’s person-centered plan. This service also includes consultation on the modifications and/or training on the use of the vehicle modifications. 9 MFP Modify Existing Vehicle for Accessibility

10 Work completed by a qualified provider who meets state requirements for automotive repair/service. Units = $4,000 Prior Authorization Procedure Code/Modifier = T2039/UA If a waiver already has vehicle modifications available as a service, MFP participants will use that service instead of the demonstration service and will bill the service using the waiver’s appropriate billing code instead of the MFP billing code.

Successful Transition Story Darby Bigford-Grisanti, Spectrum- Great Falls Supervisor served as RTC. Participant transitioned from Missouri River Manor in Great Falls in July 2015, to her own subsidized apartment in the Great Falls community. 11

Other Items RPO/CPO/Regional Manager/Regional Support Specialist Involvement Once a referral is made to MFP will collaborate with the appropriate program staff. Save the Date: MFP and AMDD will be joining forces to provide a half day annual training and a half day of Mental Health 101 training. The dates and locations are as follows: May 2 – Kalispell May 4 – Butte June 21 – Helena June 29 – Great Falls July 21 – Billings July 29 – Missoula Locations in designated towns are to be determined. MFP Demonstration Services must be tied to the participant's service plan. Resources ADRC’s can sometimes assist with Medicaid applications.
