Title of Action Plan: Brief name of your action plan Work responsibilities: Brief description of your main work Facilitation problem being addressed What facilitation problems do you have in your job that you want to solve? Objective:What will your action plan be able to achieve? Expected Outputs: What do you expect to accomplish at the end? Progress Indicators: What is it that can be measured or observed to show that the expected outputs are achieved?

Activities:HOW will you do it? Specific actions required to achieve your expected results. Time Table:WHEN and for HOW LONG will you do each activity? Inputs:WHAT resources are needed? HOW many? HOW much? From WHOM? From WHERE? Budget:What are the cost of inputs needed to implement the activities? Responsible Person/ Agency WHO will do it? Who will be your partners or team?
