Navigating to the Patient Portal

Creating an Account Use only once, in class.

Creating an Account Screen 1 Fields marked with a red asterisk (*) are required. Click Continue.

Creating an Account Screen 2 (1 of 3) Fill in required fields. Information can be edited later.

Creating an Account Screen 2 (2 of 3) Enter your email address as UserID. Write UserID and Password in learner guide.

Creating an Account Screen 2 (3 of 3) Check both boxes Click Create Account.

Completing your account Demographics Fill in required fields. “Declined to Answer” is acceptable for race and ethnicity. Click Next.

Completing your account Click Next. Insurance Entirely optional.

Logging in From now on, Enter User ID and password here. Click Login.

My Account Page Email Click E-Mail link. Enter Password. Enter new email address twice. Click Save Changes.

My Account Page Secret Question Click Password & UserID link. Click Change Secret Question Enter Password. Select new question. Type new answer. Click Update Secret Question.

My Account Page Preferences Click Preferences link. Select preferred location, provider, and email format. Click Update Your Preferences.

My Account Page Wallet Click Wallet link. To pay bills online, enter all required information. Click Add Card.

Making Appointment Requests

Appointment Requests Solutions Icons Click Schedule / Reschedule to request a new or change an appointment.

Appointment Requests Provider Select your doctor, PA, or NP from the list. Click Continue.

Appointment Requests Time, reason, and contact Fill in required fields. Additional information is optional. Click Continue.

Appointment Requests Confirm Request Check details. Do not click Submit Request during class.

Transferring Records Solutions Icons Click My Patient Paperwork to request a records transfer.

Transferring Records From another office to Heart & Vascular. From Heart & Vascular to another office.

Transferring Records To our office Fill in your information. Fill in the office sending the records.

Transferring Records Check at least one option. Type your full name, check relationship, type the current date, and click Agree. Do not click Submit in class.
