7 initial Provision of service The initial Provision of Service form is available when an IEP is marked as an initial AND you press the ‘fill from IEP/Amendment’ button on the Provision of Service form.

12 follow the kid Paperwork Required: RESA application 3-page para/aide support checklist 1 or more survey forms detailing needs (physical, academic, behavior) narrative including background, placement history, needs, par/aide’s responsibilities, evaluation plan and phase-out plan for each area of concern BIP para/aide activity log with behavioral charting as required building team data review All information must be submitted to SE at the end of the year.

14 Service Documentation FACT: A record of objectives addressed when working with students must be documented FACT: Classroom teachers can use their lesson plans for documentation FACT: For all other providers, MI Star (Zangle) includes an Objectives Services Log for your convenience

15 Service documentation The problem: documenting objectives services in co-taught classes

16 Documenting services The solution: for each student in co-taught classes -- create a log in MI Star that includes appropriate objectives print a copy of the Report to Classroom Teacher to ensure that co-teachers are aware of accommodations listed in the IEP complete the logs over the course of the marking period submit the logs to the primary provider at the end of each marking period

18 ESY is the exception, not the rule ESY is driven by goals and data Standard I: Regression/Recoupment Standard II: Nature or Severity of the Disability Standard III: Critical Areas/Stages of Learning

19 Extended school year The need for ESY must be indicated on the IEP. If it is determined that ESY is needed AFTER an IEP has been held, an amendment may be written. On-going data collection must be completed and documented on the appropriate form in October, February and April. The ESY form in MI Star must be attached to the IEP or amendment.
