Tip: ILL Contacts Ill contacts will need to have their own condition entered into WebSurv if they meet the case definition of a reportable disease. If there is no diagnostic test to enter, put a comment in the Notes field which states that the case is epi-linked to a person and include their name or the CID.

Resolution Tips CLOSED REASON: –Investigation Complete: All necessary information has been obtained and entered into WebSurv. –Unable to Locate: At times an investigation cannot be completed because the client cannot be located. Add a NOTE indicating the dates and times attempted contacts were made. –Patient Refused Follow Up: At times the client refuses to answer questions regarding the condition. Add a NOTE indicating that this took place. Include the date and time when this occurred.

Resolution Tips Closed Reason –No Investigation Required: Used when a condition being entered does not required follow up. Contact Disease Investigation staff before using this reason. –Administratively Closed: This reason is reserved for DHSS regional staff use. –Other: Use this reason when none of the other reasons apply. Add a NOTE indicating what the Other reason is.

Changing Jurisdictions –You can change the jurisdiction from your county/city to another –You cannot change the jurisdiction from another jurisdiction to your county/city as it will be read only –QA Complete “No”: changing jurisdiction will change both the Counted and Managed jurisdiction –QA Complete “Yes”: changing jurisdiction will change the Managed jurisdiction but not the Counted jurisdiction
