GOALS  Provide a useful tool to a wide variety of providers and sites  Provide a tool that will assist users in managing groups of patients.  Display RPMS information in a single GUI

DEVELOPMENT  Long wish list  Existing functionality  Phased development  User Feedback

TIPS  Don’t stress! You can’t “break” anything by creating panels in iCare  Use trial and error to create a multitude of panels to meet your needs  Name your panels in such a way you can identify the purpose  Always create a description for your panel  Share your panels with at least one other person. Anticipate vacations and time off and assign coverage.

Scenario #1 You are one of 3 Nurse Case Managers at your site and you have been tasked with ensuring that routine cancer screens are performed on your active clinic population.

TIPS  Identify your population as carefully as possible  Don’t use gender or age alone if you can avoid it  Display only that which is significant to your task

Scenario #2 You are a Family Practice Physician and have become increasingly concerned about your patients with diabetes who also have a history of heart disease. You want to be sure you are providing quality care to them routinely.

TIPS  Define your patients thoughtfully  Filter the panel until you’ve identified the group of interest for the task at hand  Use parent/child panel design technique  Know your diagnostic tag definitions  Know your performance measures

Scenario #3 You are the immunization nurse. Every day you give immunizations to the general clinic population. As an integral part of the clinic team, you want to support efforts to improve the quality of care given. You have discussed this with your supervisor and have discovered ways you can make a difference

TIPS  Identify areas you need to concentrate on  Anticipate individual patient needs prior to arrival by filtering on performance measure view  Re-establish parameters to refresh panels and over- ride outdated panels.  Assist other team members in identification of patient needs

Scenario #4 You are a public health nurse and have been tasked with supporting the local Head Start program by ensuring that their new students have all the entry requirements completed prior to the start of the new year.

TIPS  Share your panels with at least one other person  Set the auto-refresh feature at the point of panel creation

Improving Outcomes by Viewing Performance Measure Data 3 Views User Defined Panel View Aggregated Panel View Patient View
